- Negative Certificate of Criminal Records.
- Have an office to receive patients. You will need to get a result of 80%compliance or more in the facility assessment.
- Have NPI (National Provider Identifier) number.
Psychiatrists:
- Complete the hiring application in full and sign the contracts and fees that apply.
- Professional license as a Psychiatrist to practice in Puerto Rico (current).
- Have a current state and federal narcotics license (ASSMCA and DEA).
- Current professional liability insurance set to FHC with a minimum coverage of $100,000 to $300,000 (impericia insurance).
- Current membership of the College of Physician Surgeons of Puerto Rico.
- Negative Certificate of Criminal Records.
- Criminal Records.
- Have an office to receive patients. You will need to get a result of 80%compliance or more in the facility assessment.
- Have your National Provider Identifier (NPI) number.
Social worker:
- Complete the hiring application in full and sign the contracts and fees that apply
- Permanent license as a Social Worker to practice in Puerto Rico, issued by the Puerto Rico Social Workers’ Examining Board (current).
- Master’s or Doctorate conferred at a university accredited by the state where he completed his studies.
- Current membership of the College of Social Workers of PR.
- Professional Liability Insurance in force in front of FHC with a minimum coverage of $100,000 to $300,000 (Impericia insurance).
- Negative Certificate of Criminal Records.
- Have an office to receive patients. You will need to get a result of 80%compliance or more in the facility assessment.

Counsellors:

- Professional license as a Counselor to practice in Puerto Rico issued by the Board of Examiner of PR Counselors (current).
- Master’s or Doctorate conferred at a State Accredited University where you completed studies in some of the following clinical disciplines:
- Psychological counseling
- Couples and family counseling
- Mental health counseling
- Counselor in Addiction
- Professional Liability Insurance in force in front of FHC with a minimum coverage of $100,000 to $300,000 (Impericia insurance).
- Negative Certificate of Criminal Records.
- Have an office to receive patients. You will need to get a result of 80%compliance or more in the facility assessment.
