Psychologists:

  • Complete the hiring application in full and sign the contracts and fees that apply.
  • Professional license as a psychologist to practice in Puerto Rico (current).
  • Master’s or Doctorate conferred at a State Accredited University where you completed your studies:
      o

    • PhD in Psychology (PhD or PsyD)
      o

    • Master’s Degree in Psychological Counseling (MA)
  • Current professional liability insurance set to FHC with a minimum coverage of$100,000 to $300,000 (impericia insurance).
  • Negative Certificate of Criminal Records.
  • Have an office to receive patients. You will need to get a result of 80%compliance or more in the facility assessment.
  • Have NPI (National Provider Identifier) number.

Psychiatrists:

  • Complete the hiring application in full and sign the contracts and fees that apply.
  • Professional license as a Psychiatrist to practice in Puerto Rico (current).
  • Have a current state and federal narcotics license (ASSMCA and DEA).
  • Current professional liability insurance set to FHC with a minimum coverage of $100,000 to $300,000 (impericia insurance).
  • Current membership of the College of Physician Surgeons of Puerto Rico.
  • Negative Certificate of Criminal Records.
  • Criminal Records.
  • Have an office to receive patients. You will need to get a result of 80%compliance or more in the facility assessment.
  • Have your National Provider Identifier (NPI) number.

Social worker:

  • Complete the hiring application in full and sign the contracts and fees that apply
  • Permanent license as a Social Worker to practice in Puerto Rico, issued by the Puerto Rico Social Workers’ Examining Board (current).
  • Master’s or Doctorate conferred at a university accredited by the state where he completed his studies.
  • Current membership of the College of Social Workers of PR.
  • Professional Liability Insurance in force in front of FHC with a minimum coverage of $100,000 to $300,000 (Impericia insurance).
  • Negative Certificate of Criminal Records.
  • Have an office to receive patients. You will need to get a result of 80%compliance or more in the facility assessment.
  • Have a National Provider Identifier (NPI) number.

Counsellors:

  • Complete the hiring application in full and sign the contracts and fees that apply.
  • Professional license as a Counselor to practice in Puerto Rico issued by the Board of Examiner of PR Counselors (current).
  • Master’s or Doctorate conferred at a State Accredited University where you completed studies in some of the following clinical disciplines:
    • Psychological counseling
    • Couples and family counseling
    • Mental health counseling
    • Counselor in Addiction
  • Professional Liability Insurance in force in front of FHC with a minimum coverage of $100,000 to $300,000 (Impericia insurance).
  • Negative Certificate of Criminal Records.
  • Have an office to receive patients. You will need to get a result of 80%compliance or more in the facility assessment.
  • Have a National Identifier (NPI) number.

The accreditation process must be completed within sixty (60) calendar days of receipt of the fully completed application and all applicable credentials.

Good Standing must be requested by the supplier and must not be more than
180 days from the date of issue, until the date of submission to the Accreditation Committee. The document must be in addition to FHC and must be sent directly from the applicable Board of Professionals to the FHC postal address.

The office must meet the following requirements, including but not limited to:

  1. Adequate physical access
  2. Appearance and adequacy of the waiting area and the examination room
  3. Meet safety and equipment requirements.
  4. Proper maintenance of clinical records.
  5. Confidentiality of the records and any other clinical information.
  6. Access and appointment standards.

  7. Keep patients informed about the following:
    1. Rights and responsibilities
    2. Written explanation of the grievance or complaint procedure.
    3. Bachelor”s and bachelor’s degree
