  • Complete the hiring application in full and sign the contracts and fees that apply.<\/li>\n
  • Professional license as a psychologist to practice in Puerto Rico (current).<\/li>\n
  • Master’s or Doctorate conferred at a State Accredited University where you completed your studies:\n
      \no\t<\/p>\n
    • PhD in Psychology (PhD or PsyD)
      \no\t<\/p>\n
    • Master’s Degree in Psychological Counseling (MA)\n<\/ul>\n<\/li>\n
    • Current professional liability insurance set to FHC with a minimum coverage of$100,000 to $300,000 (impericia insurance).<\/li>\n
    • Negative Certificate of Criminal Records.<\/li>\n
    • Have an office to receive patients. You will need to get a result of 80%compliance or more in the facility assessment.<\/li>\n
    • Have NPI (National Provider Identifier) number.<\/li>\n<\/ul>\n

      Psychiatrists:<\/p>\n

        \n
      • Complete the hiring application in full and sign the contracts and fees that apply.<\/li>\n
      • Professional license as a Psychiatrist to practice in Puerto Rico (current).<\/li>\n
      • Have a current state and federal narcotics license (ASSMCA and DEA).<\/li>\n
      • Current professional liability insurance set to FHC with a minimum coverage of $100,000 to $300,000 (impericia insurance).<\/li>\n
      • Current membership of the College of Physician Surgeons of Puerto Rico.<\/li>\n
      • Negative Certificate of Criminal Records.<\/li>\n
      • Criminal Records.<\/li>\n
      • Have an office to receive patients. You will need to get a result of 80%compliance or more in the facility assessment.<\/li>\n
      • Have your National Provider Identifier (NPI) number.<\/li>\n<\/ul>\n

        Social worker:<\/p>\n

        • Complete the hiring application in full and sign the contracts and fees that apply<\/li>\n
        • Permanent license as a Social Worker to practice in Puerto Rico, issued by the Puerto Rico Social Workers’ Examining Board (current).<\/li>\n
        • Master’s or Doctorate conferred at a university accredited by the state where he completed his studies.<\/li>\n
        • Current membership of the College of Social Workers of PR.<\/li>\n
        • Professional Liability Insurance in force in front of FHC with a minimum coverage of $100,000 to $300,000 (Impericia insurance).<\/li>\n
        • Negative Certificate of Criminal Records.<\/li>\n

            • Couples and family counseling<\/li>\n
            • Mental health counseling<\/li>\n
            • Counselor in Addiction<\/li>\n<\/ul>\n<\/li>\n
